PRCC/FELUK (Cat): $20.00                        Deworm (Dog/Cat): $5.00

Flu - $25.00 and Rattlesnake (series of 2) (then yearly)

Flea & Heartworm Preventative will be Available

If you have 3 or more pets, to save time, you can email Lana at LonghornLana@aol.com and she can have the paperwork done ahead of time.

2. Pet info: Name, Breed, Sex, Age, Size (small, med. large), Color,

Dog or Cat (cat- short, medium or long hair) Spayed or Neutered
